Making An Appointment

Thank you for your interest in the Minneapolis Heart Institute. Appointment requests can be made by phone only at this time.

To make an appointment at our main location at Abbott Northwestern Hospital in Minneapolis, please call the Minneapolis Heart Institute at 612-863-3900 or toll-free at 1-800-582-5175.  To make an appointment at one of our other locations, please contact that location directly.

If this matter is of an urgent nature, please contact your primary physician, go to your local emergency room or call 911.

New Patients:
Before your appointment, we need information about your medical history. In most cases, these forms will be sent to you prior to your appointment. You can also print a copy of the following new patient forms, fill them out and bring them with you to your appointment.
